RN Care Manager - CA Licensed

RN Care Manager supporting senior patients across the care continuum.

• •One Medical is seeking experienced RNs with care management or population health backgrounds to join their Senior Health Care Team.
• •This role supports senior patients across the care continuum through scheduled visits and proactive outreach.
• •Key Responsibilities Conduct independent nursing visits focusing on prevention, medication reconciliation, and care plan reinforcement.
• •Support patients in managing their own care and navigating the healthcare system.
• •Coordinate with primary care providers and health navigators for high-quality, whole-person care.
• •Identify and act on open care gaps, including screenings and assessments.
• •Requirements Licensed RN in CA required. 2+ years of nursing experience in hospital, post-acute facility, or ambulatory setting.
• •Experience in care management, value-based care, or population health preferred.
• •Strong clinical aptitude and excellent interpersonal communication skills.
